Please check the Catalog before sending in a cataloging request. If there is a record in the system you do not need to send in a request using these forms.

Submitting requests for self-published titles or local publications:
For self-published titles use the notes field to indicate if the title is adult or juvenile, fiction or nonfiction. You can also suggest any subjects that could be added.
For local titles, such as documents and histories, use the notes field to indicate any subject headings you might like added. Subject headings added by cataloging will have to conform to Library of Congress authorized headings, but some general suggestions will provide a place to begin.
